Check boxes only visible on 1 of 3 my forms

Hi,

I need help with my missing check boxes.

I have three forms on my site all with the same multiple choice 6 options. However the check boxes are only visible on the first form I created. All the Form Labels are visible, but not the check boxes.

I have tried the Prevent Duplicates advise as detailed below but to no avail (Although I’m not too sure it pertain to my problem).

Prevent duplicates
Checkboxes → Settings → Optional

Prevents the form being submitted if a form has previously been submitted with the same value for this element.

Error message if duplicate found
Checkboxes → Settings → Optional

Change the error message shown when a duplicate value is found. The default is “This value is a duplicate of a previously submitted form”.

Can anyone help?
