Create additional fields as much as user press a button

I want to create a form where clubs can apply for a tournement in a certain category.
Per category it nust be possible to apply for more teams.
Of course I can create more fields then one, but i would like it more dynamic.

For example: A button which means apply for another team, and so on.

Anyway to do this?
