Duplicates issue?

I have a very simple contact form with name, email, and phone. Ajax is enabled.

When I submit the form I cannot use the same info and submit the form again. The field background turns yellow.

Do not allow duplicates is not selected for any of the fields.

I tried with no success:

– Turned of ajax
– turned off saving form submissions in WP
– cleared cache in browsers
– checked all settings

Anyone have any ideas why this is happening?
