Form do not show correctly: [raw]…[/raw]

Hello all!

I have some trouble with Quform. Simple forms without any customizing working correctly, but I made yesterday huge form with a lot of rules and bookmarks. This form is working correctly in popup widget at sidebar, but it show me a bug when I try to set it to the post as popup or ordinary form.

And show me it: [raw] Name of form [/raw].

Any ideas how can I fix it?
