Form layout in fancybox

Hello, when a form loads in a fancybox,

1. there is a large gap between the form title and the fancybox top boundary; how can this be reduced ?

2. the date fields are not aligned as expected (in one row); how can this be fixed?

3. the date picker is not functioning; how can this be corrected?

4. after the message is sent then the fancybox with the form does not disappear; it justs resets. How can I close it after displaying the “thank you” message?

I tried everything… Nothing has worked. It is a nightmare. On top I discovered that when I add a quform in the sidebar as shortcode in Siteorigin Editor then spacing are crippled. Distances between text inputs cannot be controlled.
