Form not showing

Hi,

I am trying to add a form to my page, but the form does not show. I made sure to follow all the instructions.
After pressing F12, under console I get the attached error.

I am building my own WP site, using the Divi3 theme. Please try to write a simple, easy to follow explanation, as I am not a professional developer.
