Form Not Workin as a Popup

I have the same Problem. I make form and click on this+ and after i geta code. this code i put inside the page. But after coming the website and the popup but the website is in the front and the popup is in the back.
If i go to widget and click on the QUFORM then stay there
TITLE
FORM
SHOW FORM TILEL is a hock inside
SHOW FROM DESCRIPTON is same a hock inside
ANIMATE IT CLASS
WIDGET LOGIC

if i put this code inside from WIDGET LOGIC, the popup not work.

Can you help me, how i can make sure my popup works?
