Help with "Warning: require_once"

I got this message in the page where I want the form:

Please ignore the above message, I was able to fixed it.
What I did was delete the Quform folder from the server and upload it again.
Not sure what happen or why but it worked for me.
Now, the question that I have now is how to configure the email that I receive?
For example, right now I can ONLY GET:
Name
Email address
Message
IP address

But I have other fields,
WHERE CAN I ADD THOSE FIELDS?
and take out the ip address?
