How can I build a part number with form?

I need to build the quotation form where all chosen values of each field would build the part number at the bottom of the form.

Something similar to the following:
=======================
Field 1 = 23
Field 2 = AB
Field 3 = 75C
Field 4 = 34
Field 5 = WW

PART #: 23-AB-75C-34-WW
======================

Any suggestions/advice, please?
