quform2 bug after update – date field & blank form edit

Hi

We have updated to your quform version 2; however, we have noticed two bugs:

1. The date field is not showing up – mixed up – see attached snapshot or you can see it on our site at http://www.southafricasafarishotelstravel.com/quform2

2. We cannot edit the form – the form edit page is blank – see snapshot. Its the same for all our forms

Kindly advise how to fix this issue.

Hi Ally,

I tried the Rebuild script cache, and it did not work for both problems. Performance tab – Datepicker script was enabled by default.

I sent you the logins via email

We use your quform plugin on 5 sites so we need to get this working.
