Retrieve Data from Database

I use Quform for WordPress so I bought it for a recent PHP implementation and I have the data being emailed and saved into a database. I can go into phpMyAdmin and I can see the data but I cannot retrieve it without error.

There must be a simple PHP script to connect to the database so I can display the data on a web page but I don’t see anything in the documentation.
