Theme template messing up form. – any suggestions

Hi, Thanks a million for the great plugin, i hope to use it on future projects.

I have a query. i am using the form on one of my websites but the theme css seems to be messing up my form, particularly my date picker. see link

This form shows up perfecty in preview and using the twenty twelve theme.

Have you any suggestions to correct this? Thanks a million.

Thanks for the reply. I disabled the plugin as i wanted to use the license on the live site. I ended up installing another wordpress installation inside the main one and customising the twentytwelve theme to suit the form. I had to do it that way as I’m doing 16 sites at the moment and under pressure to get them done asap. They are happy with it so it’s okay. 🙂
