2nd Form appears to be invisible and have [raw][/raw] wrapping it

I created my first form with no problem, When i created my second form and inserted it i am getting a big problem. The form appears to be taking up space on the webpage, but is invisible, it is also wrapped with the [raw][/raw] tags.

I duplicated the first form when i started the second form as it had a lot of the same fields.
