Able to send form without any data

If you go to https://www.liquidvpn.com/ and use the create ticket button and just click submit the form will submit without ever entering any data even know there is a lot of required fields. As soon as you put a field in to that drop down you have to follow the forms rules.

Go to your Form and remove the Value of “Select Department ”

Your Drop down should look something like this

then the drop down gets set at just a few pixels wide and that looks ugly. Is it possible to make the width fixed and if so how?
