Add a data line via button.

First of all hello,
my English is not so good. So I took some pictures to show my problem.

I would like the fields “Player Name, Player No., Size, Important and Color” to be added at the bottom when you click the “add/add row” button and an X to delete is offered. See pictures.

PS I created the pictures of what it should look like using Photoshop.
