Added Quform shortcode, broke javascript on page

For some reason when I added

<div style=”clear:both;width:100%;”>
[iphorm id=”1″ name=”form-name”]
</div>

To the content area of the page it breaks the page( http://shusterman.com/intake-secure-2.html )

Any reason why? Can’t seem to find the fix, and the form isn’t showing.

I’m showing that jQuery is indeed on that page. Is there a specific script that quform is loading?
