after latest update, page loads twice??

i have multiple step form
page1 – redirects to page2 – redirects to page3 – redirects to thank you

so for somereason and it started AFTER i updated quform plugin (from 3 versions old to newest)
i think i had 1.481 ?

anyway,

on page3 and when user submits form, the page refreshes and then redirects?

NOTE: i have Facebook pixel tracking on… and the checker says the pixel is loaded twice?

so technically this is happening (from a url point of view)

this is WHAT SHOULD HAPPEN (as user fills out the forms)

/page2.html
/page3.html
/thankyou.html

HOWEVER, this is what is happening

/page3.html
