Ajax submit not working, no JS error messages being reported

My site, torontoheadshot.com is no longer ajax submitting the form on the main page. It is opening a new window to submit.

I have disabled all caching and minification plugins as well as disabled Cloudflare CDN services. Still no go.

Please help, I am stuck at this point and I cannot be sure if potential customers are emailing me and it not being sent.
