Allign name and last name text

Hi, in the first field Nombre y Apellido (Name and last name) if you see the text are not allign with the others below let me know how i can fix that?

And how can set up when the people send the form, appear other page, message but not still in the form page.

And how set up each form to send to a different email adress.
