"An error occurred Refresh the page and try again" on iPhone and iPad

I have QuForm WordPress 2.5.0 plugin installed, and created a contact form for my website. I am able to fill out the contact form and submit it successfully using Safari on my iMac desktop computer. I receive an email with the information which I entered on the test submission. However, when I open up my website on my iPhone or iPad using Safari and try to submit the exact same information as a test, I get this message “An error occurred Refresh the page and try again”. Any suggestions on what I need to do to successfully submit the contact form using my iMac desktop, iPhone and iPad? Thanks!
