I have big gaps between my form elements. I’ve reviewed the FAQ fixes and tried them all including adding the code to the functions.php file. My test page is at

    http://dev12.ravecommunications.com/contact/

I’ve placed the form 4 times on the page: form shortcode only, form shortcode in raw shortcode, form shortcode in a code tag, and form shortcode in a div tag. The red text on the page identify the different code that follows.

I’m using a StudioPress child theme called Optimal. Is the theme causing the gaps or am I doing something wrong? I’ve used Quform on a number of other sites using different themes and it always works as expected.

A test of the form shows it’s working correctly.

Thanks for any help you can provide!
