Blank Form Edit Page Using Avada Theme

We recently updated to the Avada Theme. Now getting a blank page (HTTP ERROR 500) for the form edit link only. Everything else is working. Using Quform 1.10. WordPress memory set to 256M.

All Disable Script output boxes are unchecked. Fancybox is enabled.

Any ideas how to fix this problem?
