Cannot Save Changes to Form

Hi, I have adapted a form based on your Job Application template. It imported successfully and then I have made a number of changes successfully and saved them. I am not sure what I did, but now I am unable to save the current set of changes.

Any suggestions as to how I can troubleshoot this?
