Can't get it to work with my own form

Am trying to get it to work with my own form layout, but its just doesn’t seem to work. I’ve followed instructions but to no avail.

the current form which has no data base, i want to convert and use it with your script.

Hello, i don’t no what i did but its working now.

But am having other issue and can’t solve it. If for example i leave a field empty and try to summit the form, its takes me to a new page

” Please go back and correct the following errors
Message
This field is required
Type the word
This field is required”

Rather than do that is there a way the errors can be produced on the form(same page)?
