Checkbox selected automatically?

Hello, I use a plugin “CSS3 Web Pricing Tables Pack (Grids)” to sell packages of my product. I’ve created a form to my product and inside it a checkbox field to the packages of my product, is there a way to when someone click on the package, goes to the forms and the checkbox packages fields selected automatically???

And is there a option to create a single line field and show the name of the product????
