Completed form – need to click submit for each image

Hi there, I have completed a form and added it to my site.

However, I seem to have to click on submit for each file upload then click submit one they are upload.

I have copied this form from another site which worked fine.

Any thoughts??

Not sure but maybe those are 4 upload fields and you need only one file upload field then go to settings and there you can allow how many files to upload .
