Conditional form not showing.

Got a conditional form not showing – all other quforms show – even in visual composer.

I’ve checked and the correct wp footer code is in place and I receive no errors it just shows a [raw] tag

just tested and it works fine if its loaded in the template file

not really a solution for a client though
