Conditional Logic

Hi Ally

I have an issue on forms with conditional logic not working since last update. I have four or 5 forms on various sites.
Try this site as an example

All fields should be hidden until a user selects wither cake enquiry or general enquiry. Depending on choice different fields appear. This has always worked on all forms but now stopped.

All sites running the latest Avada theme and php 7.2 All on latest WP release and QForm
Could you please advise asap. I can send site log in details if required through direct email.
Important to get these working at this time of year especially

Avtually the whole form layout has gone

I have taken 2 screenshots for you. One with v2.9.1 and one with v2.9.3
2.9.1.png is how it should look
2.9.3.png is how it looks with latest update

I have reveretd back to 2.9.1 on all sites. Let me know if you need access
