Create a form

Hello,
I installed the plugin today and wanted to create a form right away.
Unfortunately, I always get the error message “An error occurred adding the form
Failed to insert into database, check the error log for more information”

I’m using WordPress 6.0.1 and the Avada V7.8.1 theme
