Create “Rating” Form

What options do I have in a QuForm2 to make one of the forms section where person can rate 1-5 each for each answer.

Eg…

What do you love to eat the most?
Rate 5=highest, 1=lowest

5 Pizza
3 Cookies
2 Fruit
1 Chocolate
4 Hamburgers

…and those numbers 1-5 could be changed around but the text (Pizza, Cookies, Fruit, etc) would stay permanent and not be able to be changed.
