Creating Dropdown menu extend option

Hi Team,

First thing i really thanks for such a great software !!

I need help regarding creation dropdown menu if example there is 2 option YES or NO and someone select YES and i need further information how can i add there single line text so there explain in details there and if the select NO its remain hide.

please check picture what i need .
