Creative a patient intake form – quiz style

Hi I have a client who wishes to have a form on their website for patient enquiries – however, using the conditional logic, the form will end with a suggestion of the service they need.

Is it possible to create a kind of quiz style form where depending on the answers given the patient will have an end screen with the suggested service?
