custom menu items that link each page of built form

HI,

I’d like to create a sidebar with custom menu items that sort of links each page on a form. So for instance if I have 10 pages built and I’d like to see page 5, I’d like to be able to click on the page 5 menu item that takes me to the page.
I’ve attached a screenshot of what I have in mind.
How can I accomplish this?
