Data from checkboxes doesn’t submit via e-mail.

Hi,
I have a problem with data collecting in my form.
One subpage (LOKALIZACJA) have some checkboxes.
They showing with conditional logic.
When user fill the form and send it this data isn’t showing in e-mail and collected in database.
I of course switched on there options:
“Show in email” and “Save to database”.
Could you check it, please?
It’s very important for my client.
Exported form is attached.
