default values in ie and checkbox disappeared

Hi,

I’m using your awesome form here: http://www.impuls-werbeagentur.at/wordpress/ at the end of the page.

I have 3 strange problems:
1. The checkbox left to the text “Ich wünsche einen Rückruf unter der angegebenen Nummer.” is not showing
2. We use the default value to write the names of the fields in the textboxes but they are not shown in IE
3. I have required fields. Unfortunately qform accepts the pre filled values and submit the form, even if the user didn’t fill out the fields.

Please help me!
