Different Quforms To Different Mailchimp Lists

I have followed the instructions to successfully add the inputs from one Qufom to a specific Mailchimp list.

I would like to add inputs from different Quforms to different Mailchimp Lists, and I prepared the attached code…

The second form is not registering the inputs… Can you please check where I am going wrong???
