Display Entries on a WordPress Site

Hello,

I am looking for an easy way to display entries from a specific form on my wordpress site. I work with animals and I would like to be able to use the form as a Lost & Found. Ideally the user could submit the form and their name and other form info like the picture would display on the Lost/Found page.

Sorry if this has been posted already, but I searched and couldn’t find anything.

Any help is appreciated!
