Error editing form (new form for each item)

Hello,

I use your plugin from a while. I have some forms in my site. But this days I have a problem editing actual forms. When I add new items and save, the form remains the same but I have a new form (without name) for each item.
I tried to make a new form but I get the same error… new form for each item.
An example form, from my site:

I want to add more items/products on that form/page, but I can’t.

Hello,

I tried your plugin with each other plugin and the problem was with ‘Translator Revolution WP Plugin’.
Now when I need to change a form, I disable the ‘Translator Revolution Plugin’ and after that I activate back.
