Error for no apparent reason?

I’ve ran a horde of tests on my from and I can’t reproduce the error some of my visitors are getting when using the form.

I rely heavily on Quform but get an entry once every few weeks. The entries are important and it takes some work to fill it out so it would be great if we could figure this one out so my visitors(customers) don’t get frustrated and move on.
