Errors With Simple Radio Button Form

Copied InLine Radio button code from all elements
Copied working email and submit button from Contact form
Using process.php
Error messages attached.

Need some help to determine how to get this simple rb form working. Thanks!

I understand now the reasons for the errors. Using email and send has several expectations whereas I thought I was sending the results of the radio button fill in. Was trying to cobble up a simple form wherein user selected rb and supplied their email address.
