Fatal error on line 11 "CustomAdminSubPage"

when I try to activate QuForm I get the following error:
Fatal error: Class ‘CustomAdminSubPage’ not found in …../wp-content/themes/paradies/functions/admin_options/AdminPageFactory.php on line 11

Can you help me for this?
