First time setup questions

Hi everybody,
I just purchased QForm plugin and I’m struggling to setup some things.

1) I would have the label settings like this form ICON fixed on the left like this form https://lombardia.depuratori-acqua.eu/form/

2) I would make the form more vertically compact and with the submit button centered like this page https://lombardia.depuratori-acqua.eu/form/

3) I would have the first field preselected (like the user select the field to start completing the form)

How can I setup this?

4) I setup a dropdown menu with some value and for each value I send the data to a different recipient, I would like to have the recipient in the form submission title. how can I do?
