Form disappeared

Hello,
I was using the plugin normally in a test Server. When I moved the project to my client’s server, it stopped working. I can’t edit the form i already created.
Here is what happens:

The browser console gives this error:

I tried reinstaling and updating, but nothing worked.
Could you help me?

Hi Ally,

I already tried exporting the form and it did not work. Actually, even the one in the test server stopped working. I tried to deactivate all other plugins to see if there was some kind of conflict but that did not work also. I can’t create new forms or edit the old ones, the interface just disappears as shown in the first image above.

Is there a way to fully delete the plugin from my database so I can try a fresh new install?
