Form doesn't appear in admin

Hi,

First: GREAT plugin! I have the unlimited license and I use the forms on various websites.

But this time, it doesn’t work well. When I add a form, it doesn’t appear. There is no form found. I have completed the plugin/theme conflicts steps, but the problem still remains (also with another default theme and with the deactivating of all the plugins).

The problem occurs on this website: http://www.imagoconcept.nl.

Hope you can help me! I really want to use quform on my website.
