Form Gone Wacko on me…

I changed the customers theme and now my form renders terribly in all browsers. FYI the theme is Inovado. Can anyone give me quick tip where I should start…this is a conditional form I learned and created last year…hate to have to start from scratch again. Here is the link to the IMAGE
