Form not displaying in Chrome

I followed the troubleshooting sections regarding forms not showing. I do have the required code in the footer and have made sure there’s no extra space and and believe no more than one jquery library loading. I am receiving a different error in the console (only when the form is added to the page, otherwise it doesn’t throw an errror):

Uncaught TypeError: undefined is not a function:
