Form not submitting.

I have a form that is not submitting at all. When you hit send it just does the little loading animation forever.

I tried Firefox, Chrome, IE 10, and Safari, same result in all of them.

All of my plugins are up to date, I am using Envato, Issuu PDF Sync, Jetpack, Revolution Slider, Theme test drive, and Under construction.
