Form not submitting

I have multiple forms on our site and all are working, except one. This form claims to submit (see attached) but no entries are shown on the back end: https://fernandoawards.org/nomination-form/

I realized the “Save Submitted Form” check mark under the form’s general settings was not turned on. It is working now.
