Form Nothing Like Preview

I know the theme CSS can interfere but my preview: here

Looks nothing like the finished result: here

Why is there a massive space between the intro text at the top and inbetween fields?

The form just looks awful – any ideas?

Remove the <pre></pre> tags around your form and see if that helps
