Form styling perfect on pages but wrong on posts.

My form is setup and styled just perfectly on my pages.:

However the same form inserted into a post has different styling and even the quform hidden field is showing:

http://www.jules-photographer.com/travel/india-photography-tour-2014/

Thanks Ally for your reply, I forgot that my theme uses a different header for that category! I have included the links and all is working perfectly. Your reply triggered the solution so thanks for your insight.
